                 INTRODUCTION 
                
                The Asian Disaster Preparedness Center (ADPC) is currently
          conducting a study in the developing countries of Asia and the Pacific
          to document the existing capacity building efforts on disaster
          management. The purpose of the study is to identify, document and
          assess the effectiveness of capacity building activities underway and
          recommend additional activities that will enhance the capacity of
          developing country institutions and individuals to effectively respond
          to disasters.  
                This study would be presented in the regional workshop expected to
          be organized in May 2005 in Thailand. Selected successful capacity
          building initiatives would be showcased at the workshop. The workshop
          would review the effectiveness of the strategy of disaster management
          and recovery and also outline specific steps that can be taken to
          increase capacities of institutions in Asia to better respond to
          humanitarian emergencies.  
           
          More details and explanation regarding compiling the details of the
          capacity building efforts is elaborated in the Explanatory
          Notes presented below. We would be grateful if you could
          provide us with the information requested through the survey forms
          attached.  
                
                EXPLANATORY
      NOTES  
      There are 2 survey Forms presented in one file accompanied by an
      Example (Table) attached that provides guidance in completing the information requested.  
       
      Form A: Introduction 
      A simple form seeks basic information on the respondent name and contact details and the number of capacity building initiatives since the year 2000.  
       
      Form B: Reporting Sheet 
      The reporting sheet is a structured format Form in MS Excel. We request you to kindly fill in the details of the capacity building initiatives, where your organization or your country has been involved. For each
      program/project, the following information is required;  Name of the organization, Name of the
      program/project, Start and finish date, Location and geographical coverage, Type of support, Funding source, Funding amount, Implementation agency, Objectives, Approach, Key activities, Target outputs/outcomes.

      In order to guide the respondents in compiling information we have also provided
      Table 1 (click to open) that presents an example of a capacity development
      program. We would appreciate if you could be as concise as possible.

 Please  download the file (reporting-sheet.xls)
          and compile the information in the Form A and B (To
          save the file, point your mouse on the link and right click, and click
          save target as. A new window will open and save the file in your
          preferred folder).

 The Forms and Table (example ) in MS Excel are included in one file but in different
           worksheets. After you open the MS Excel file, you can easily see different worksheets at the bottom.
